Steps to a Healthier U.S.

Client: Centers for Disease Control and Prevention (CDC), Coordinating Center for Health Promotion (CoCHIP), National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP)

The Steps program is a U.S. Department of Health and Human Services (HHS) program advancing the HealthierUS goal to help Americans live longer, healthier lives. Through funding of community-based interventions, this program aims to reduce the burden of diabetes, obesity, and asthma, and address three related risk behaviors – poor nutrition, physical inactivity and tobacco use and exposure.

The CDC needed an infrastructure to provide technical assistance support to 44 local communities to ensure the successful implementation of the Steps to a HealthierUS Cooperative Agreement program. Through this project, Constella designed a technical assistance plan that includes communications, program planning, program evaluation, and training for local grantees. Constella is now implementing this plan for the CDC.

Prevention Program Branch Technical and Administrative Support Services

Client: Centers for Disease Control and Prevention (CDC), Coordinating Center for Infectious Diseases (CCID), National Center for HIV, STD, & TB Prevention (NCHSTP), Division of HIV/AIDS Prevention (DHAP), Prevention Program Branch (PPB)

The PPB provides program leadership, assistance, and funding for state and community programs that provide HIV prevention services. PPB's HIV prevention programs are currently run by over 230 grantees, including 65 state, local, and territorial health departments, and over 168 community-based organizations.

In order to accomplish its overall mission effectively, PPB has an ongoing need for a broad range of administrative and technical support services on a long-term basis. As a sub-contractor to Z-Tech, Constella's role is to provide technical and administrative support services to the HIV Prevention Program Branch for three tasks: Resource Inventory, Records Management, and Training.

Resource Inventory. PPB requires an on-site resource inventory for storing all guidelines, guidance, major speeches, program announcements, and other documents and videos. These resources are available to requestors within 24 hours or less. Constella's operational resource center includes a comprehensive and up-to-date online and hardcopy list of all 1,300 resource materials that are available on site.

Records Management. Constella provides filing and file maintenance services on site at the NCHSTP campus for filing and tracking all documents pertaining to the official grant files of PPB grantees. During the past three years, to further PPB's goal of a paperless grants management system, Constella has completed scanning more than 200,000 documents into electronic PDF format and is inputting these documents into a custom designed, Web-based document management system.

Training. Constella provides assistance in developing, identifying, and delivering training, including assessing project officer skills and competencies, conducting and scheduling group and individual employee training, and reporting on training delivered.


Rapid HIV/AIDS Testing Program Managers Training Curriculum

Client: Centers for Disease Control and Prevention (CDC), Coordinating Center for Infectious Diseases (CCID), National Center for HIV, STD, & TB Prevention (NCHSTP), Division of HIV/AIDS Prevention (DHAP), Capacity Building Branch (CBB)

Rapid HIV Test technology is one of the most exciting developments in recent years to encourage the estimated 850,000 to 950,000 Americans who are unknowingly infected with HIV to learn of their infection and to seek treatment, care, and support. With antiretroviral therapies becoming more available, affordable, and easier to take, the chance for people living with HIV/AIDS to remain healthy for long periods of time is now more real than ever before.

The first step to accessing treatment, care, and support is for individuals to learn their HIV status. People who know their status are likely to protect themselves and prevent further transmission to others. Indeed, the CDC estimates that the largest percentage of the new 40,000 infections occurring in the United States stem from people who do not know their HIV status. Despite advances in treating and living with HIV/AIDS, barriers to learning one's HIV status persist.

Access to testing and the often-discouraging time lag between having the HIV test and learning of the results are two of those barriers. Rapid HIV Test technology dramatically reduces those barriers by making high-quality HIV testing portable, reliable, faster, and easier to administer. Venues never before considered for HIV testing can be used, community-level personnel can be trained to perform testing, and results can be offered within minutes. These are great strides for encouraging people to know their HIV status and, therefore, prevent new infections while accessing life-enhancing treatment, care, and support.

The Development of Curricula for Program Manager's Rapid HIV Test Training project began in September 2004. By accessing and applying proven principles of adult learning theory to ensure effective instructional design, Constella has worked to develop this training curriculum. These trainings include all training materials—posters, slides, handouts, facilitator's guide and training manual.
